Welfare Reform Team - Support

The Welfare Reform team will deal with anything money-related (debts, the cost of moving house, furniture assistance, upfront costs for rent/deposit, hardship payments etc.). They provide a personalised programme of support based on the individual’s circumstances and there is no time-limit for this support.

The patient will be allocated a case worker who coordinates the programme of support through other agencies and organisations and they take on the responsibility to ensure that the support is delivered to the patient.

It starts off with finances/debt etc. usually, but it isn’t solely focussed on the patient’s financial situation (e.g. the case worker may refer to drug & alcohol services and will ensure that the patient has attended).

Complete the referral form sent by Sammi from NHSMail ONLY.
