Social Prescribing

Social Prescribers work closely with GPs to assist patients with their non-medical needs. We do this by helping them identify, 'what matters to them'. Working collaboratively to create a care plan, which may include referrals into local services and organisations within the community.

Our hope is for pateints to have a better quality of life. Ultimately supporting them to improve their overall health and wellbeing, via connecting them to community groups and statutory services for support. For example; social groups like games club,  gardening clubs, sports programmes and much more.

This youth social prescribing service supports young people aged 11-18 who are experiencing emotional health and wellbeing issues (eg. feeling stressed, worried, lonely) from the following GP practices:

Usually replies in 3 days
60% of referrals accepted
Service access criteria: Aged 11-18 presenting with low level anxiety, low mood, confidence, self image or social difficulties that are impacting on their life. Not receiving CAMHS service and no severe mental health diagnosis. If referral meets criteria for CAMHS, please discuss with us before making a referral to social prescribing.
