Mental Health Practitioner

Mental Health Link workers are health professionals based in GP practices. We provide counselling for low level mental health (anxiety, depression, stress) This mental health supports the following GP practices: Goodcare Practice, Broadmead Surgery, Jubilee Gardens Medical Centre, Somerset Medical Centre, West End Surgery and Yeading Medical Centre.

  Brief Mental Health Interventions

-          Provide advice and guidance to GPs

-          Support GPs with the gap between MINT service

-          Working with other ARRS such as Social Prescribers and Clinical Pharmacists

-          Practice case finding

-          SMI and CCMI reviews where appropriate, promoting staying well plans/referral

 

Referral Criteria:
-          Referee need to make it clear why they are referring patient, is it for SMI, depression review, medication concerns(not effective)/anxiety, depression,  etc.

-          Patients who require a comprehensive assessment

-          Patients referred to IAPT, not engaging or IAPT declined referral

-          Patients whose Mental Health needs are utilising too much GP time

-          Drugs and alcohol abuse to be referred to RISE for specialist support

-          Depression and anxiety, medication not making a difference *non urgent*

-          Clients not meeting MINT Criteria

-          First mental health presentation low risk (not psychosis)

-          Urgent to be seen but not in a crisis other primary care services considered

-          Discharges from other mental health services if need be

-          Review declined referrals with no clear pathway from other mental health services

 

Exclusion Criteria:
-          Patients under the care of the MINT service

-          Patients who have been referred to MINT, awaiting an appointment

-          Patients who have an urgent need, refer to SPA

-          High risk patients e.g. active suicidal ideation
