Social Prescribing Coffee Morning - 3 Middlesbrough PCN's

Holgate, Central and (Greater PCN tbc) joint coffee morning. Currently held at Lothian Rd community hall, but gaining user feedback to aid decision making for future venues. 

-Has worked with Holgate PCN or Central PCN social prescribing and referred/invited by link worker only
-Has come to the end of their SP journey OR is working towards it, and the coffee morning would provide a beneficial transition, part of exit strategy 
-No longer needs ongoing 121 support to be able to attend indefinitely (i.e. link worker may attend first session or so but cannot be committed on to attend long term) 
-Full risk review has been completed before inviting to first session. Link worker feels SU is suitable to attend a group session and will not be disruptive or pose a risk to self, others or environment 
-SU understands that the session is for social interaction with peers, and not an appropriate forum for seeking professional support or raising concerns or personal issues in that environment. Of course, if they have any concerns or need further support then they can speak to the facilitators privately for guidance/signposting 
-All attendees agree to uphold respect to others, and treat anything shared with them as confidential and without judgement 
-If any attendee shows behaviours that the facilitators feel are not benefitting them or the group, they may be asked to leave
