British Red Cross - Assisted Discharge Service - UHP (Derriford)

A service to help people return home after time spent within UHP (Derriford) Support is up to 72 hours. 

Dedicated, trained support staff and coordinator's will work with patients once referred via NHS teams to understand the support required to enable a successful return home., Our support is not medical but has an holistic, person centred approach to support people. Activities' can include (but are not limited to)

* Support on the ward prior to discharge

* supported transport home

* Shopping, settling in, prescription collection

* return to outpatient appointments, attend community medical appointments.

* Signposting, advocation support

Referrals are received via internal NHS teams at Derriford (UHP) We can support anyone over the age of 18 who reside within the Plymouth post codes.
