Healthier You - NHS Diabetes Prevention Programme

Take control of your health and prevent type 2 diabetes with the Healthier You NHS Diabetes Prevention Programme.

This free, nine-month programme is designed to help you reduce your risk of developing type 2 diabetes through expert-led support, practical advice, and healthy lifestyle changes. Whether you prefer group sessions, digital support, or remote coaching, there’s an option to suit your needs.

  • Group Programme: Join face-to-face sessions at local venues, meet others on the same journey, and learn together about nutrition, movement, and motivation.
  • Digital Programme: Access the Second Nature app for personalised support from your Health Coach, in-app features, and hundreds of delicious recipes.
  • Remote Programme: Receive support via Zoom or telephone calls, with tailored options for clients with visual or audio impairments, women with a history of gestational diabetes, and non-English speakers (courses available in Urdu, Punjabi, and Bengali).

Eligibility: Adults at risk of developing type 2 diabetes, including those who have received a letter, text, or email from their GP. Register online to get started.

Cost: This service is free and funded by NHS England.

Locations: Available in Cornwall and the Isles of Scilly, Cambridge & Peterborough, Derby & Derbyshire, North East London, North West London, South East London, and South West London.

Service access criteria:

Be aged 18 or over. Has ‘non-diabetic hyperglycaemia’ (NDH) identified by blood test within the last 12 months. Registered with a GP in City & Hackney, Newham, Tower Hamlets, Waltham Forest, Barking & Dagenham, Havering or Redbridge Non-Diabetic Hyperglycaemia (NDH) HbA1c of 42 - 47.9mmol/mol (6.0%-6.4%), Fasting Plasma Glucose (FPG) of 5.5-6.9mmol/l If the patient has a history of Gestational Diabetes Mellitus (GDM) then HbA1c can be below 42 or FPG below 5.5
