
Adult Weight Management
The Adult Weight Management pathway is a 12-week intervention, consisting of two 60 minute sessions per week. These sessions will be delivered by Health Improvement Lifestyle Officers at Northgate Arena, Ellesmere Port Sports Village, Winsford Lifestyle Centre, and Northwich Memorial Court. Each session will include physical activity, educational content, body composition assessments, nutritional education, and goal-setting activities.
Weight Management Eligibility Criteria:
BMI 30 - 39.9, 18+ years of age
Must also be diagnosed with one of the following conditions:
Learning disability
Severe mental illness (SMI) limited to: PTSD, bipolar disorder, schizophrenia or psychosis
Hypertension (prescribed antihypertensive medication)
Physical disability (a person’s physical functioning is clinically evidenced as having a long-term negative effect on their ability to exercise, and they are in receipt of a qualifying disability benefit related to their physical function)
