Social Prescribing - Arc Bucks PCN

Social Prescribing is a free and confidential service that focus on ‘what matters to me’ and taking a holistic approach to people health and wellbeing. Social prescribing link workers work together with you to help set and achieve goals to improve your overall outlook.

In conjunction with your local GP, council, other professional and volunteer organisations in the local community, a Social Prescribing Link Worker can offer support to navigate these services with you, finding the best way forward for you at that time.

Your Social Prescribing Link Worker can set small goals to help support changes that you can make, this may be short term by signposting to services that will benefit you, or up to a maximum of 12 weeks.

Some of the things we can help with are:

• Housing, benefits, social care and financial support and advice.
• Employment training, volunteer education and learning opportunities.
• Healthy lifestyle advice and physical activity, arts, music, outdoors and creative activities.
• Befriending, counselling, and emotional support groups.
• Volunteering opportunities.
