Exercise at Home

Exercise at Home is a home-based programme tailored to meet the needs of individuals who might be housebound, live with chronic conditions, have mobility and balance issues, and find Activities of Daily Living challenging. 

After initial consultation and assessment with a qualified exercise practitioner, an individual-tailored programme and session plan will be created. Each participant will be supported for 8 1:1 visits followed by a final consultation to assure further support.

The sessions are prescribed by qualified exercise practitioners and led by trained volunteers, however please note that they are not physiotherapists or medical doctors and cannot diagnose illness, prescribe medication, or perform manual therapy.

The programme is not a substitution for Community physiotherapy rehabilitation sessions, or any uncompleted rehabilitation programmes prescribed by the GP/other medical professionals but best placed to follow the intervention.

 

Exercise at Home Sessions include:

·      Initial assessment and consultation

·      Evidence-based Prescribed Exercise Sessions

·      1:1 support

 

Fees:

·      Assessment, consultation and exercise prescription - Free of charge

·      Round of 8 1:1 sessions - £80

·      Round of 8 1:1 sessions with low incomes/pension credit discount - £40
