Our Dementia Care Navigation service is here to support older people and their families on the Isle of Wight who are living with dementia or facing mental health challenges.
Our dedicated Older Person's Mental Health Navigators offer personalised support to help you maintain independence, improve your wellbeing, and navigate the health and social care system. We listen to what matters most to you and work together to create a tailored plan that addresses your daily challenges.
- Emotional support for you and your family during difficult times
- Help accessing social and community activities to stay connected
- Domestic support and advice for help around the home
- Information on falls prevention and home safety, including fitting minor aids and equipment
- Ongoing point of contact for you and your family, with or without a dementia diagnosis
- Signposting to relevant services and working alongside Adult Social Care and The Memory Service
Visits can take place in your own home or a location convenient to you. We also host Dementia Chatty Brews at community venues across the Isle of Wight, offering opportunities for conversation, engaging activities, and expert guidance on topics like Attendance Allowance and Blue Badge services.
