Care Coordinators are Health Professionals based in GP Practices, who work closely with GP’s alongside Social Prescribing Link Workers and Health and Wellbeing Coaches. This role is part of the NHS’s long-term plan to make personalised care the ‘norm’ across primary care. The idea is to help support GP Practice staff in helping people look after and manage their own physical and mental health.
Bereavement Support Care Coordinator’s do not provide patients with clinical support, Care Coordinators listen and signpost people to services in their time of need.
Bereavement Support Care Coordinator role is to help people who have been directly or indirectly impacted by a bereavement e.g. Patients who has recently had a close family member/friend pass away, bereavement/mourning of a job, pet etc.
Bereavement Support Care Coordinator’s alongside providing a listening ear also signpost patients to services and groups for example ‘Let’s Talk’ bereavement counselling, ‘Bereavement Groups’, ‘Coffee Groups’ and ‘Community Groups’.
