Pendle East Primary Care Network (PCN) Neighbourhood Team
You can make a referral to the Pendle East Neighbourhood Team for support with a range of health and wellbeing needs. Our team works collaboratively to help individuals access the right services and support in the community.
We offer the following services:
- Social Prescribing – Connecting you to community groups, activities, and practical support to improve your wellbeing.
- Mental Health Wellbeing – Providing emotional support and guidance to help manage mental health concerns.
- Health and Wellbeing – Offering advice and assistance to help you improve your overall health and lifestyle.
Please note:
We are not an urgent service. Once a referral is received, our team meets once weekly on a Tuesday to review and allocate referrals to the most appropriate service. You will then be contacted by a member of the team following this process.
To refer into this service, you must be registered with one of the 5 Pendle East GP surgeries, these are The Richmond Hill Practice, The Pendle Medical Practice, Harambee Surgery, Barrowford Surgery and Barnoldswick Medical.
