Warwickshire Community Health Parkinson’s Disease Nurse Service

PARKINSON’S DISEASE NURSE SPECIALIST SERVICE

Our service provided specialist advice, support and education and is a point of contact for patients, carers and healthcare professionals.

We offer a specialist Parkinson’s disease assessment for patients and carers, with symptom and medication management. We also make referrals and signpost and liaise with other services.

The above services will be provided by domiciliary visits / telephone / clinics.

 

Notes:

· We are not an emergency service

· We will not be able to provide a service for patients with primary dementia.

· We will not be able to provide a service for patients with vascular Parkinson’s, essential tremor, drug induced Parkinson’s

· Service hours are Monday – Friday 9am to 5pm

· All referrals via iSPA or post.

Service access criteria: 1. Patient has confirmed diagnosis of idiopathic Parkinson’s / Parkinson’s plus disorder 2. Patient is under the care of a Parkinson’s specialist. 3. Patient is registered with a Warwickshire GP. 4. Referral to include named of PD specialist and GP with date of diagnosis, current medication including times with any recent changes, past medical history and if patient has any problems with communication.
