Children and Young Adults with Disabilities Service - Under 18

CYADS supports families in Harrow with children and young adults aged 0-25 who have moderate, severe, or profound disabilities, complex health problems, or significant support needs.

Our dedicated team of social workers and occupational therapists work with you, your child, and other professionals to assess needs and develop a tailored package of support. This service is designed for children and young people who require more help than universal services can provide.

Support may include:

  • Personal care
  • Accessing structured activities in the community
  • Play schemes
  • Short breaks
  • Direct payments

If your child is not eligible for CYADS, we will help you access other suitable support, such as targeted or specialist services through Early Help, short breaks via the Core Offer, social work assessment through Harrow’s First Response Team, or signposting to universal services.

Transition planning for ages 18-25: Our 18-25 team helps young people with learning disabilities prepare for adulthood, supporting a smooth transition to adult services.

Eligibility:

  • Live in Harrow
  • Aged 0 to 25
  • Need significant support from another person or equipment for tasks like personal or night-time care, eating, or taking part in activities
  • May have moderate, severe, or profound learning disabilities (including PMLD), severe physical or sensory disabilities, complex health needs, autism and/or ADHD (with additional criteria), or attend special schools

How to refer:

  • Parents and carers can refer by contacting Harrow’s MASH Team (see contact details below)
  • Professionals can complete the online referral form

After referral, we assess your child’s needs, considering their daily life, family impact, strengths, wishes, caring responsibilities, community support, benefits, and input from professionals. We may also offer a Parent Carer Assessment.
