Children and Young People Social Prescribing service for East Merton, Morden and North Merton, focused on improving child healthy weight and low level emotional health and wellbeing outcomes.
Service access criteria:
Programme area: East Merton, Morden and North Merton PCNs
Ages 13-19 years old (or up to 25 years old if a young person has additional needs)
Registered with a GP Practice in East Merton PCN, Morden PCN, North Merton PCN OR living in or attending school in CR4, SM4 or SW192 areas.
Reason for Referral:
Living with a high level of obesity categorised as ≥ 98th percentile on BMI scale OR/AND
Living with a low level emotional or mental health issue
To provide a children and young people’s social prescribing programme in East Merton, Morden and North Merton, where social prescribing link workers will support children and their families/young people to achieve their health and well-being goals.
Patients must be aged 13 – 19 or up to 25 if they have additional support needs and live, work or go to school or be registered with a GP in CR4, SM4 or SW192 to access the service.
