The Barking & Dagenham, Havering and Redbridge Falls Clinic is a multidisciplinary service delivered across the three London boroughs of Barking and Dagenham, Havering and Redbridge. The team includes a Falls Medical Consultant, Physiotherapist, Occupational Therapist, Nurse, Falls Practitioner and Integrated Care Assistants. The service provides assessment and management for adults aged 50 and over (unless medically proven at risk of falls) who live in BHR and who have three or more risk factors, such as a slip/trip or near fall in the past year, five or more medications, balance problems, or difficulty rising from a chair of knee height. The aims are to prevent falls, reduce the number of slips and falls and related hospital or A&E admissions, minimise fear of falling, and support independence and quality of life, while educating patients and carers on coping strategies. What happens in the clinic includes medical reviews and medication optimisation by the Falls Consultant, routine tests and health advice from the Falls Clinic Nurse, strength and balance exercises led by the Physiotherapist (both in person and virtually), and home assessments by the Occupational Therapist to identify risk factors and suggest home adaptations or equipment. Integrated Care Assistants support clinic functions and can assist with home visits to maintain safety in daily life. Referrals can be via self-referral or through GPs, consultants, community matrons, A&E staff, community therapy teams, nurses, social services and other professionals.
