Wellbeing for Warwickshire Community Links Services

If someone you know is experiencing social isolation and/or would like one-to-one support to access and engage in the local community, then this service may be able to help.

We can offer you one to one support to engage in your local community and access the things that are important to you.

This service is open to you if you are feeling lonely or isolated, or if you just need some help to access your community. Our friendly and sociable team will visit you in your home and then support you to get out and about and start making connections.

We might start by helping you to book and attend appointments with your local GP, or other healthcare services. We could help you join a group where you can make new friends or learn a new skill. Whatever you decide you will lead on your support. Everything we do will be based around your aims and needs.

We will work with you for up to six sessions to help you make a plan, to build your confidence, then access and engage in your local community.
