NHS CPFT Community Rehabilitation Service

The NHS CPFT Community Rehabilitation Service offers specialist support for adults (18+) who are housebound due to physical illness, injury, disability, or infirmity. Our team of physiotherapists and occupational therapists will visit you at home to help you live independently and safely.

After an initial assessment, we work with you to create a personalised rehabilitation and treatment plan tailored to your needs, circumstances, and goals. This may include:

  • Enabling programmes to boost confidence and independence
  • Exercise programmes to improve mobility and strength
  • Information and education about your condition to help you manage it better
  • Housing needs assessment
  • Mobility aid, assistive equipment, and home adaptation assessments

How to access: For Community Occupational Therapy, you can self-refer by calling 0345 045 5202, or a carer, partner, or family member can refer you. Healthcare professionals can also make referrals. For Community Physiotherapy, a referral from a healthcare professional is required. If you are unsure which therapy is best for you, please speak to a healthcare professional.

Once referred, you may receive a telephone call to discuss your needs and expectations. You will then be placed on a waiting list and given an appointment in due course. Our therapists will visit your home in uniform with an identity badge, spend time understanding your goals and concerns, and develop a plan that supports you and your carers.

Falls Prevention: If you are over 65 and have had a fall at home or while out, ask your GP or health professional for a referral to CPFT's Falls Prevention Service.

Cost: This service is free. Financial support for home adaptations is available for those on low incomes, subject to eligibility and discussion.
