Social prescribing is a vital service that helps patients improve their overall well-being by connecting them with non-medical resources and community-based support. Our social prescribers work closely with GPs, community organisations, and patients to address the social factors that impact health, such as loneliness, housing, employment, and lifestyle choices.
How can a Social Prescriber help?
- Support for social, emotional, and physical wellbeing
- Signposting to weight-loss programs, exercise classes, local activities, and clubs
- Help with substance misuse and gambling
- Advice on debt, housing, and volunteering opportunities
- Loneliness and elderly support groups
- Connections to a range of charities and local organisations
Social prescribing empowers you to take control of your health, encourages self-management, and can reduce reliance on medication. There is growing evidence that social prescribing leads to improvements in quality of life, emotional wellbeing, and levels of depression and anxiety.
