Care Navigation from Age UK Redbridge, Barking & Havering is a free service designed to support older people living with one or more long-term health conditions in Redbridge, Barking & Dagenham, and Havering.
Our Care Navigators work closely with you to understand your circumstances, hobbies, interests, and goals. Together, we create a personalised plan to help you access the support and services you need. This may include:
- General information and advice
- Support to access local services and activities
- Help with benefits advice and information
- Signposting for legal advice
- Support to access transport schemes
- Connecting you with social groups and activities of interest
Our goal is to help you enjoy a good quality of life, build confidence, and maintain independence—whether that means getting out and about, taking up new activities, or enjoying hobbies you once pursued. For those less able to leave home, our volunteers offer companionship and help open up new opportunities for social interaction.
Who can use this service? Anyone living in Redbridge, Barking & Dagenham, or Havering who is experiencing social isolation or managing multiple health conditions. Referrals can be made by GPs, professionals, family members, or by contacting us directly.
Cost: This service is free to eligible clients.
