This free community health check is for people aged 30 to 74 who live in Wokingham and/or are registered with a GP in Wokingham. It helps you understand your health and what next steps may support you to stay well. - Helps identify potential health issues early - Offers an accessible community alternative where a GP NHS Health Check may not be available - Focuses on helping you understand your health and next steps - May suit people who have not had a health check within the last 5 years - Not for people already receiving treatment for conditions such as diabetes, high blood pressure, heart failure, high cholesterol or chronic kidney disease Cost: Free How to access: Request a referral through Joy.
