RISE (Rutland Integrated Social Empowerment Service) is part of Rutland County Council's neighbourhood approach to supporting people through social prescribing and other methods.
Social prescribers are health professionals based in local GP practices. They work closely with GPs to help patients address non-medical needs by identifying what matters to them and creating a personalised care plan. This may include referrals to local services and community organisations.
RISE covers the following GP surgeries:
- The Uppingham Surgery
- Oakham Medical Practice
- Empingham Medical Centre
- Market Overton and Somerby Surgeries
