Our Community Diabetes Service is here to support adults living with Type 1 and Type 2 diabetes in North Kent. We understand that managing diabetes can be challenging, and our dedicated team is committed to helping you take control of your health.
What we offer:
from clinical nurse specialists and care support staff. - Education programmes for both Type 1 and Type 2 diabetes, including X-PERT (Type 2) and T1DE (Type 1) courses.
- Flexible appointments at our clinics or, for housebound patients, home visits.
- Personalised care plans to help you manage your diabetes independently and reduce the risk of long-term complications.
- Close collaboration with your GP, Practice Nurse, and consultant to ensure comprehensive care.
Type 2 Diabetes – X-PERT Education: Attend 6 sessions (2½ hours each) over 6 weeks at Sheppey Community Hospital or Sittingbourne Memorial Hospital. Morning, afternoon, or evening courses are available, with follow-ups at 6 months and 1 year. Meet others with Type 2 diabetes and learn about lifestyle and diet changes to slow progression.
Type 1 Diabetes – T1DE Education: Attend 4 full-day sessions over 4 weeks. For those on a basal bolus insulin regimen (4x daily insulin). Learn to adjust insulin doses based on carbohydrate intake and activity levels.
Home Visits: Available for patients who are housebound, either temporarily or permanently. If you can visit your GP practice, you will be expected to attend a clinic appointment.
Service hours: Monday to Friday, 8am to 5pm.
How to access: Referrals can be made via your GP, Practice Nurse, or by contacting our Care Coordination Centre. Download and complete the referral form from our website and email it to VCL.DGSS-CCC@nhs.net.
Areas served: Dartford, Gravesham, Swanley, and Swale.
