Dementia Link Worker

Are you experiencing memory loss or waiting for a memory assessment?

Greater Peterborough Network can provide a Dementia Link worker to offer information and advice through 1:1 appointments to yourself, and if needed, someone who supports you.

This service is available to you if you are registered  with a GP practice within Peterborough and Cambridgeshire. The Dementia link worker can provide  signposting to services and organisations to help with your health and wellbeing.

Flexible appointments to suit your needs, either over the telephone, within your own home or in a community setting. 

Referrals can be made by telephone, email or filling out the referral form,

Working hours for the Dementia Link Worker service are Monday to Friday, 9am-5pm.
