Care needs assessment
Care and support for adults when they are unable to complete day-to-day tasks because of a physical disability, a learning disability, a long-term health condition, frailty due to old age, mental ill-health or substance abuse.
Care and support is also available to carers who care for an adult friend or family member and this has an impact on their own wellbeing.Care and support can include:
- Support, home adaptations or home care to help you to remain independent in your own home
- Help to access a suitable care home, when living at home is no longer an option
- Temporary support, like reablement or rehabilitation after an accident, illness or stay in hospital
