The Frailty Home Treatment Service (east Kent) provides individualised, acute medical care for frail patients experiencing a health crisis, delivered directly in their own home or care home. Our goal is to help patients avoid unnecessary hospital admissions by offering expert assessment, diagnosis, and treatment in familiar surroundings.
Led by consultants from the Community Frailty Team and East Kent Hospitals University NHS Foundation Trust, our team includes advanced clinical practitioners, nurses, and therapists working closely with GPs and hospital discharge teams. We operate as a virtual ward, ensuring patients remain under the care of a lead clinician throughout their treatment.
