Service access criteria:
Referral criteria
• Aged 18 years or over
• The service user must live or be registered at a GP practice in Wolverhampton.
• Confirmation that appropriate assessment of motivation, confidence and ability to enact change has been completed.
• Service users who smoke tobacco
• Would like to receive additional support to stop smoking
• Service users who are not receiving a stop smoking intervention from another provider
Exclusion criteria
• Under 18 years of age
• Receiving a stop smoking intervention from another provider
