Long-Term Health Needs Service

What is our service?

Our service supports Children and Young people living in Medway and Swale who have; Diabetes, Epilepsy, Asthma and other Long-Term Health Needs.

We devise support programs and activities that the Children and Young people co-create.

We work with a number of Children and Young people to be advocates for their peers.

Referral Criteria:

 Young person or Child is aged between 0-18 (without SEND diagnosis)

Young person or Child is aged between 0-25 (with SEND diagnosis)

Young person or Child has been diagnosed with Diabetes, Epilepsy, Asthma or Other Long-Term Health Needs

Professionals/Families/Self referrals

Lives in Medway or Swale
