WorkWell Wyre Forest Health Partnership PCN

The WorkWell programme provides holistic, personalised support to individuals with health conditions, helping them remain in or return to employment by bridging the gap between health and employment services and addressing health-related barriers to work. The programme is a collaboration between Department of Work and Pensions (DWP) and the NHS.

 

Goal: To address health and employment-related challenges, ultimately improving well-being and reducing unemployment due to health issues.

 

How it Works: The WorkWell programme works by connecting participants with a Work and Health Coach, who conducts an initial assessment to understand their health needs and employment challenges. Based on this, the coach creates a personalised development plan, which may include coaching sessions, signposting to external services, or referrals to a Multidisciplinary Team (MDT) for specialised support. Participants typically receive ongoing coaching over several sessions, where the coach provides guidance on managing health conditions, adjusting in the workplace, and facilitating a return to work or employment retention.

 

Who’s it for: The WorkWell programme is for individuals who are experiencing health conditions—both physical and mental—that are affecting their ability to stay in or return to employment. Participants must be eligible to work in the UK and reside within Herefordshire or Worcestershire area or be registered with a GP in that area.
