Patients should of been seen by a clinician, have all appropriate GP investigations/assessments/tests initiated, have all relevant referrals (eg safeguarding, mental health, Village Agents, ASC, CRS, DNs) been made prior to this referral and registered within Tone Valley Primary Care Network
