Everyone Active’s Exercise Referral Scheme is designed to help people with long-term health conditions become more active and improve their overall wellbeing. The programme provides a safe, welcoming environment for individuals with underlying medical conditions or those at risk of developing them, supporting long-term behaviour change through regular physical activity.
What does the scheme offer?
- A tailored 12-week programme to help you self-manage your condition with confidence.
- Initial appointment with an Exercise Referral instructor to discuss your needs and preferences.
- 6 and 12 week reviews to support your progress and help you stay motivated.
- Activities may include walking sports or groups, chair-based exercise, swimming, group exercise classes, water-based activities like Aqua Fit, and gym-based exercise.
- Social sessions to help you meet others and build connections.
Who can join?
- Adults (16+) who are inactive and have a low or moderate risk long-term condition, such as asthma, arthritis, high blood pressure, diabetes, obesity, depression, anxiety, osteoporosis, or are recovering from Covid-19.
- If you have been regularly active for more than three months (two or more times a week), you may be recommended alternative membership options.
How to access the scheme:
- Speak to your GP, nurse, physio, or other healthcare practitioner for a referral.
- Once referred, you will be contacted to arrange your initial appointment.
Cost: This is a chargeable scheme. Prices may vary per site or activity, with pay-as-you-go and direct debit options available.

If an Individual presents with any of the following contra-indications to physical activity then our Exercise referral specialists will contact the referring practitioner to discuss the referral before accepting:
Instable Angina
Systolic blood pressure >180 and/or diastolic>100 mmHg
BP drop> 20mmHg demonstrated during ETT
Resting Tachycardia > 100 BPM
Uncontrolled atrial or Ventricular Arrhythmias
Unstable or Acute Heart Failure
Unstable Diabetes
Febrile illness (temperature over 38 Degrees)
