What are social prescribers?

Social Prescribers work in GP practices and are part of the NHS, they are here to:

  • Support you with social and emotional needs that affect your health and wellbeing, such as loneliness, housing or money.
  • Help you to connect with community activities, groups and services or charities

The Social Prescriber will contact you to help you achieve your health and wellbeing goals.

What do Social Prescribers do?

Social Prescribers are here to help you with non-health related problems, including, but not limited to:

  • Loneliness and isolation
  • Housing and homelessness
  • Money problems and benefits guidance, information on charities
  • Support for older people, carers, disabilities
  • Help to buy food and to access food banks
  • Low level mental health support (such as anxiety, panic attacks, low mood and stress)
  • Work related issues, help to find a job and volunteering
  • Support with addictions, by signposting to drug, alcohol or gambling service
  • Help using computers or getting internet access
  • Domestic abuse

The important thing is that Social Prescribers focus on what matters to you. They create a safe space for people to talk without judgement.

Social Prescribers can refer you into local services that can help you with the problems you have.

What happens after referral has been received?

The referral will be screened and allocated for the Social Prescriber to contact on phone number provided in the form. Response time varies, as per patient demand, but patients are usually contacted within 1-2 weeks.

Please confirm patient is an adult (18years old +), does not have an urgent or severe mental health needs and is currently registered with one of the following practices:
