Location: Our day centre is based in Wolverhampton, and we also have an office in London. Services are primarily delivered in-person at our Wolverhampton hub.

Service Account Eligibility Criteria.
to be eligible for a service account with life beyond horizons individuals must meet the following criteria :
1 :Age Must be aged between 16 and 30 years (adjust age range as appropriate for your service ).
2: Diagnosis
must have a diagnose learning disability and oral spectrum condition.
3: Support Needs
Requires assistance or support in one or more of the following areas developing independent living skills engaging safely in the community and Social Educational or recreational activities
4: Consent
The individual or the legal guardian if applicable must consent to, engage with life beyond horizon services participate in an initial needs assessment comply with service agreements and safeguard and policies
5: Community engagement
Willingness to participate in community based activities designed to promote social inclusion and independence.
6:Risk Assessment
able to participate safely in activities with reasonable adjustments any high risk behaviours must be manageable within the service staffing and safety structure.
7: Fund and Referrals
must have appropriate funding in place for example personal budgets direct payments local authority funding will be self funded .
referral can be made by family social workers educational institutions or self referral wear appropriate .

Community integration: support an access to local resources, clubs ,and groups.
Independent living skills: promote and daily living Skills like cooking budgeting travel training and self-care.
Activity -Based support :Engaging in meaningful activities such as arts, sports ,volunteering and educational workshops.
Personal development: build confidence, communication skills and life skills.
