How do I start?
Upon referral - you will be contacted and sent a self-booking link to book in for a your health check. Your case maybe discussed with your local primary care team to provide you with the best support possible within your community.

Upon referral you agree for your medical records to be shared with your primary care team at Healthy Hyde PCN. They will access your medical records to provide ongoing support with your health needs and none of your personal information will be shared to any third party.
