We understand how important it is for people to return home following their hospital stay as soon as they are medically fit to do so. That's why we work with hospitals and alongside people in hospital to enable their discharge. We support individuals to facilitate their hospital discharge and for up to 6 weeks following their return home from hospital.
We accept hospital referrals and self-referrals from people who have an upcoming hospital stay or in the 7 days following their discharge.
If you have had a stay in hospital, you may require additional care and support when you return home. Reablement refers to the care and support people access following their discharge from hospital. The type of care and support that is provided, depends on the individual’s situation, their health and wellbeing goals, and their personal preferences.

That is why we have conversations about discharge as early on in your journey as possible. If your hospital stay is planned, we can even arrange your discharge support before your stay!
