Move Together Buckinghamshire is a free programme designed to help residents aged 50 and over, living with a long-term health condition, to become more active and improve their physical and mental wellbeing.
Funded by Buckinghamshire Council’s Public Health Team and delivered by Active in the Community (AITC), this initiative offers up to 12 months of personalised support to help you move more in a way that suits you, with professional guidance every step of the way.

Meet our referral specialists:
- Nicola – Personal trainer and running coach, specialising in safe, effective exercise for all abilities.
- Kane – Cancer Rehabilitation, Obesity & Diabetes, and Exercise Referral Specialist, dedicated to helping people with long-term conditions lead active, independent lives.
- Lavender – Doctoral researcher and Level 4 Cardiac Rehabilitation Specialist, expert in tailored plans for chronic conditions.
- Vicky – Cancer Rehabilitation and Clinical Exercise Specialist, supporting people with long-term health conditions to build strength and fitness.
Open hours: Monday–Friday, 8:30am to 5:30pm
- Over the age of 50 years old
- Currently inactive or fairly inactive (do less than 30 minutes of activity a day)
- Motivated to increase activity levels to improve their health and wellbeing.
- Living with a long term health condition (list of eligible conditions below)
- Resident of one of the 17 registered GP practices below or live in an opportunity bucks ward (check your postcode by clicking here)

- Alzheimer’s
- Arthritis and Musculoskeletal Conditions
- Asthma, COPD (and wider lung conditions)
- Cancer
- Dementia
- Diabetes (controlled)
- Hypertension
- Limited Mobility Conditions
- Chronic Fatigue Syndrome
- Mental Health Conditions
- Multiple Sclerosis
- Osteoporosis
- Parkinson's
- Stroke (stroke must have occurred more than 3 months ago)
- Falls (for people at risk of developing frailty)
- Heart Conditions
Exclusion Criteria
- Unplanned or unexpected admission to hospital because of a health condition in the last 4 weeks. If this is the case, seek advice from a healthcare professional before joining.
- Acute coronary syndrome (a heart attack within the last 6 weeks or unstable angina)
- Severe or poorly controlled asthma
- Severe COPD
- Currently accessing cardiac or cancer rehab or pulmonary services
- Currently undergoing wider tests regarding the long term health condition(s)
- Blood Pressure >180mmHg (systolic) or >100mmHg (diastolic)
- Resting heart rate >100 bpm
