Our social prescribing link workers are here to help you find the right support for your health and wellbeing.
We work with a wide range of patients, including:
- People living with long-term health conditions
- Individuals experiencing low-level mental health issues
- Anyone feeling lonely or isolated
- People with complex social needs
How it works: You’ll have regular conversations with a dedicated link worker (typically 6-12 sessions over three months). Together, you’ll explore activities, groups, and services in your local community that can offer practical and emotional support to help you get back on your feet.
Self-referral is easy: Patients can refer themselves to our social prescribers using the online form available on our website.
