The role of the Neighbourhood Carers Lead is to support earlier identification and coding of informal carers and to connect them to locally based health and well-being activities and support. This helps enable them to:
- Protect and promote their mental, physical, social and financial wellbeing
- Continue in their caring role (if desired)
- Enjoy a life outside of their caring responsibilities.
An individualised and person-centred approach is used to provide support including the use of What Matters to Me conversations, Personalised Care and Support Planning and signposting.

The individual needs to be registered at one of the seven GP practices forming Ely North and South Primary Care Networks.
