The Learning Disability and Autism Liaison Nurses support adults who are in a Buckinghamshire Hospital or who are due to have a planned admission or appointment. This is not a community service, but a referral can be made for support during a planned future admission.
o We support adults in Buckinghamshire hospitals who have a diagnosis of learning disability and /or autism
o We can support with emergency admissions, planned appointments and procedures
o We offer support during hospital stays
o We offer practical support and advice and support for families and carers
o We provide support with decision making, communication and when planning to go home
o We provide resources to support procedures and appointments
o We can support services in preparing areas to allow people to access and have their health needs met.
o The learning disability and autism liaison service is available Monday to Friday 8am-4.30pm, excluding bank holidays.
- The person being referred must be an inpatient in a Buckinghamshire Healthcare NHS Trust hospital, or has a planned admission, procedure or appointment
- This service does not support patients in the community unless it is to prepare for a hospital visit
- Referrals can be made by patients, their family members, carers, friends and any paid professionals working with the person.
- The service supports adults with a confirmed diagnosis of learning disability and/or autism.
