Service access criteria:
Patients must be over 18 and either have consented to the MDT referral or it is a best interest decision.
Be registered within Tone Valley Primary Care Network (Taunton Vale Healthcare, Warwick House Medical Centre, North Curry Health Centre, Lyngford Park Surgery or Creech Medical Centre).
All relevant referrals have already been actioned eg safeguarding, mental health, Village Agents, ASC, CRS, DNs.
The MDT is a forum to share information between teams and to come up with a shared approach to support patients, who will benefit from a multidisciplinary team approach.
