Havering Child Health Hub Pilot

Havering is a pilot initiative aimed at enhancing the health and wellbeing of children and young people in Havering.

The pilot fosters collaboration among a wide range of professionals including General Practitioners (GPs), Nurses, Paediatricians, Social Prescribers, School Nurses, Health Visitors, Social Workers, and other healthcare and community professionals.

By promoting trust, mutual learning, and shared decision-making, the virtual Hubs in selected practices strengthens coordination and delivers more integrated, responsive care for children and families.

Service access criteria:

Resident registered with a GP practice in Liberty, Crest, North and Marshalls PCN. 

Aged 0-18 or up to 25 with SEND.

Children and young people who would benefit from a multi-disciplinary discussion. 

 

Exclusions: Patients requiring emergency multi-disciplinary team discussion.
