Service access criteria:

Open to all Brent residents, the Brent Health Matters health inequalities programme holds regular events across the borough, taking health services out into the community. The programme also has a grants scheme, a health educator programme, a clinical, mental health and Children and Young People's teams.

Events are co-produced with community partners and target diverse and disadvantaged groups where health inequality issues are more prevalent. Initiatives such as our health educator service peer support diabetes programmes, our community health pods, our case management support and our visits to community, faith and business settings to carry out health checks, one-to-one mental health consultations and more have proved valuable and impactful. 

You can call the Brent Health Matters Advice Line, Monday to Friday 9am to 5pm, on 020 8102 3456 (choose option 4) to ask any non-clinical questions about health and social care and you will be signposted and supported to access services.
