Age UK Wandsworth - Hospital Discharge Service

This service operates to assist with the transition home following a stay in hospital and prevent re-admission.  Our service co-ordinator will initially meet with the recently discharged person to discuss their requirements and offer information on any additional services or care requirements.  A DBS-checked and trained volunteer is then selected to visit the individual on a weekly basis. Our volunteers are able to help with jobs around the house, do some shopping, encourage gentle exercise, collect prescriptions and also signpost further support if required.
