Lifestyle Medicine Service - Sevenfields PCN

What Is This Service?
This program is designed to empower you to take charge of your health and wellbeing by making small, sustainable lifestyle changes that lead to long-term benefits. Grounded in the principles of lifestyle medicine, the approach focuses on six essential areas that work together to support a healthier, more balanced life:

  • Nutrition – Develop healthier eating habits that nourish your body and support overall wellness.
  • Physical Activity – Incorporate regular movement into your routine in a way that’s enjoyable and achievable.
  • Restorative Sleep – Improve the quality and consistency of your sleep to enhance recovery and mental clarity.
  • Mental Wellbeing – Build resilience, reduce stress, and improve your emotional health.
  • Social Connection – Strengthen relationships and foster a sense of belonging and community.
  • Reducing Risky Behaviours – Identify and minimise habits or choices that may negatively impact your health.

Rather than offering a one-size-fits-all solution, this service meets you where you are and supports you in making practical changes that fit your lifestyle.

Open to anyone over the age of 18 and registered with a Sevenfields GP practice, this service is particularly beneficial for those with diabetes, poor sleep, hypertension, raised lipids, raised BMI and stress.

 
How Does It Work?
Over a 12-week period, you’ll be guided and supported by coaches through a combination of one-on-one and group sessions. These sessions are designed to help you set realistic goals, track your progress, and stay motivated along the way.

Whether you're looking to boost your energy, manage stress, improve your fitness, or simply feel better day-to-day, our coaches will work with you to create a personalised plan and provide the accountability and encouragement you need to succeed.
