Falls Prevention
If you’re eligible for the service, you’ll receive up to 25 weeks access to our Falls Prevention classes, designed to improve your strength and balance. Classes are available at a number of community venues across the borough and are completely free. Our instructors will talk you through everything and even provide you with exercises you can do in your own home too.

Service access criteria:

Resident of Cheshire West and Chester or are registered with a CWaC GP practice. Aged 65+, identified as at risk of falling. 

 

This programme is intended for patients assessed as low to medium risk of falls (Rockwood Scale 2–4). You can review the Rockwood Frailty Scale here: https://www.england.nhs.uk/south/wp-content/uploads/sites/6/2022/02/rockwood-frailty-scale_.pdf 

 

Entry Criteria – Patients must be able to:

· Stand unsupported

· Walk at least 20 feet unaided

· Rise from a chair without assistance from another person

 

Exclusion Criteria – Patients with the following conditions are not eligible:

· Unstable angina

· Heart attack within the past 3 months

· Abdominal Aortic Aneurysm (unless the GP confirms fitness via the referral form)

· Uncontrolled blood pressure

· Uncontrolled diabetes (consistently above 9 mmol/L)
