my mhealth is proud to partner with Kent and Medway ICB to provide free access to the my mhealth app for people living with long-term conditions who have a suitable diagnosis. 

 

About the my mhealth app 
The my mhealth app is a free digital platform designed to support people to manage their condition more confidently, stay as healthy as possible and reduce the risk of unplanned hospital visits.  

Widely used across the NHS, the platform offers a range of self-management tools, symptom monitoring and structured educational courses that can be accessed from home at a time that suits the individual. 

 

Who can use the my mhealth app? 
Anyone registered with a GP practice in Kent and Medway who is living with one of the following conditions can access the my mhealth platform: 

  • Asthma (myAsthma)
  • Diabetes (myDiabetes)
  • Heart disease (myHeart)
  • Chronic obstructive pulmonary disease (myCOPD) 

 

How to get the my  mhealth app? 
You need to complete the self-referral form to have an account set up for you. You will then receive an activation email with step-by-step instructions on how to set up the app.

The my mhealth app is available for anyone with COPD, Asthma, Diabetes, or Heart Disease who is registered with a GP practice in Kent and Medway. myAsthma is available to patients aged 13 and above. All other my mhealth modules are suitable for patients aged 18 and above.
