Social Prescribing Link Workers help people connect to activities, groups, and services in their local community to meet practical, social, and emotional needs. This free service is available to adults registered with GP surgeries in Cambridge.
Link Workers are part of the Personalised Care Team at your GP surgery. They work with you to address health and wellbeing challenges that can't be solved by medicine alone, such as housing, finances, relationships, unemployment, and loneliness.
- Who can benefit? Social prescribing is especially helpful for people with one or more long-term conditions, those needing support with low-level mental health issues, people who are lonely or isolated, or anyone with complex social needs affecting their wellbeing.
- How does it work? You'll have a friendly chat with a Link Worker, either in person, online, or by phone. They get to know you and help with practical concerns (like debt or housing) or social isolation (connecting you to local groups and activities that match your interests). The focus is always on your individual needs and concerns. You won't be pressured, but you'll be encouraged and empowered.
- Access: Anyone over 18 can make an appointment via their GP surgery. You don't need a GP referral, but your GP may suggest it if they think it would help. Link Workers often work across several GP surgeries in Cambridge.
