
H4All's Wellbeing Service aims to help the wider population that need support to better manage long term health conditions, frailty and social isolation.
All referrals are triaged through our Single Point of Access who will investigate any concerns clients may have and offer appropriate and timely information on issues such as access to services, aids and adaptations, social clubs and activities, and care needs. The triage team can signpost clients to appropriate services and help them to identify whether they need further interventions, including allocation to a Wellbeing Support Officer for further intervention.
A Wellbeing Support Officer (WSO) can arrange a home visit if people have been identified as needing further intervention and support. They will complete an assessment form to help tailor support to the client's needs. WSO's can offer client's emotional support and the time they need to speak comfortably about issues that are important to them, work with clients on practical solutions and link them to appropriate services.
H4All CIO has access to a wealth of local and national knowledge to enable people to secure their rights and entitlements, make well informed choices, avoid problems in the future and to help self-manage their own health and wellbeing.
Clients must:
• be 18 years and over
• be registered with a Hillingdon GP and/or living in the London borough of Hillingdon
• Having difficulty managing one or more long-term health conditions or at risk of their condition exacerbating
• Have complex social needs which affect their wellbeing
Important note:
* Clients must be made aware of and consented to a referral
* Clients have an identified social prescribing need
* Clients must NOT have severe mental health or substance misuse needs and/or actively in-crisis
