Care Navigation

Care Navigation from Age UK Redbridge, Barking & Havering is a free service designed to support older people with multiple health conditions across Redbridge, Barking & Dagenham, and Havering.

Our experienced Care Navigators work closely with you to understand your needs, interests, and goals. We start with an initial conversation to get to know you, including your hobbies, wishes, and any worries you may have. Together, we create a personalised plan to help you access the support and services you want.

  • Information and advice: We help you understand and access local services, including benefits advice, legal support, and transport schemes.
  • Social connection: We can signpost you to social groups, activities, and opportunities to meet others, helping to reduce isolation and build confidence.
  • Support for independence: Our service aims to help you enjoy a good quality of life, encouraging you to take up new activities or revisit old hobbies, whatever your health needs.
  • Volunteer support: For those less able to get out, our volunteers offer companionship and help you connect with your community.

Who can use this service? The Care Navigation service is available to older people living in Redbridge, Barking & Dagenham, and Havering. Anyone can refer a new client—just call us or email to get started.

Cost: This service is free of charge.

Barking and Dagenham, Redbridge and Havering residents 50+. Must have at least two long term health conditions.
