West London Works – IPS in Primary Care

West London Works Programme is designed to support people who have a common mental and/or physical health condition to achieve: 

·         Meaningful, competitive, and sustainable employment if they are not employed. 

·         Sustained employment, in their current role, if they are already employed. 

 

Using the Individual Placement and Support (IPS) model, a dedicated IPS Employment Specialist will work with the participant and their Health and Social Care Professional to provide a bespoke service tailored to their preferences and choices into sustainable employment. 

We will provide comprehensive support that includes CV and interview preparation, confidence-building, wellbeing support, and job search aid. Our goal is to assist individuals in securing a fulfilling job that matches their preferences and ensuring they maintain this employment.  

Once the participant is in work, the Employment Specialist will provide them in-work support which will include regular confidential reviews as well as liaising with the employer on reasonable adjustments and in work support. 

Our IPS Employment Specialists will be based in their designated borough for a minimum of three days a week and be on hand to provide in person updates and support to referral partners.

If someone is over 18 years old and meets the following criteria, they are likely to be eligible.

 

Eligibility:

- Be a resident of Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow Hillingdon or Hounslow

- Be registered with a GP (the surgery does need to be in the boroughs listed above)

- Be prepared to self-declare a mental and/or physical health condition. 

- There is no requirement to be in receipt of Universal Credit or any other benefit
